Voter Registration

To register to vote in Wisconsin you must be an United States citizen, at least 18 years of age, lived in the municipality for which you are registering for at least 28 days, and not be a convicted felon still required to report to a probation or parole officer.
  • The registration form (linked below) requires your Wisconsin driver’s license number. If you do not have a Wisconsin driver’s license, you must then provide the last four digits of your social security number.
  • You will need to provide proof of residence in the City of Mequon. Acceptable proof of residence must contain your current and complete name (both first and last name), and your complete residential address (house number, street, city, and zip code).  If your document has an expiration date, it must be valid on election day. You must provide State approved proof (see below).

You can register online at http://myvote.wi.gov, if your valid State of Wisconsin Driver License or State of Wisconsin ID card issued by the Wisconsin Division of Motor Vehicles (DMV) includes your current City of Mequon address.

Examples of acceptable proof of residence include:

  •  A current and valid State of Wisconsin Driver License or State ID card.
  • Any other official identification card or license issued by a Wisconsin governmental body or unit.
  • A real estate tax bill or receipt for the current year or the year preceding the date of the election.
  • A university, college or technical college identification card (must include photo) ONLY if the voter provides a fee receipt dated within the last 9 months or the institution provides a certified housing list to the municipal clerk.
  • A gas, electric or telephone service statement (utility bill) for the period commencing no earlier than 90 days before Election Day.  This cannot be a private service.
  • A bank or credit card statement.
  • A paycheck/paystub.
  • A check or other document issued by a unit of government.
  • A letter on public or private social service agency letterhead identifying a homeless voter and describing the individual’s residence for voting purposes.
  • A residential lease that is effective on date of registration (must provide entire document). Not valid if registering by mail.
  • An intake document from a residential care facility such as a nursing home or assisted living facility.

This is not a comprehensive list, but categories of the most common types used by voters.
